Many University schools or departments require or encourage their students to complete internships or clinical placements with outside facilities as part of their degree requirements. Most of these placements require a contract or an affiliation agreement between the facility and the University as a condition of accepting our students as interns or trainees.
A good starting point in finalizing an acceptable agreement is to use one of our standard affiliation agreement templates. Affiliation agreements should be in Microsoft Word format and submitted to our office for review using the Affiliation Agreements form.
